Fees and Services
It is your responsibility to contact your insurance company to make sure we are an approved provider for services under your insurance. You should also check on the necessity for authorization of our services. Many insurance companies require prior authorization for services. Look for the MENTAL HEALTH CUSTOMER SERVICE NUMBER on the back of your insurance card.
We agree to bill your insurance as a courtesy but ultimately, you are responsible for payment for all services. All co-payments and deductible amounts are due at the time of service. At any time during treatment, should you become ineligible for insurance coverages, you must contact us with the understanding that you will be 100% responsible for all charges incurred. Questions regarding your insurance coverage, benefits, copayments and deductibles should be directed to your insurance company.
All services not covered by insurance such as: letters, forms to be filled out, evaluations, extended phone calls, or consultations with schools or other providers will be directly charged to you at an hourly rate.
As a courtesy to other clients wanting to be seen for appointments, any missed appointments or cancellations with less than two full business days notice will be charged a $75.00 fee for the visit. This fee will be directly billed to you. If you miss your initial appointment without at least two full business days notice, you will not be able to schedule another appointment until this first (missed) appointment is paid for.
ACCEPTED FORMS OF PAYMENT
We accept checks, cash, VISA or Mastercard credit or debit cards for payment of services.
There are many benefits of private pay including higher levels of confidentiality and available assessments and evaluations that are not typically covered under most health insurance plans. If you would like to learn more about this option, please contact your provider directly.
